Ensures optimal health for members by collaborating with health team and community resources. Evaluates members in their home on an ongoing basis and communicates information to the team, member, and payor. |
| Qualifications: |
Required: Bachelors Degree in Nursing with one year community nursing experience or Associates Degree in Nursing with 3 years community nursing experience. Must have reliable transportation to work sites and a valid drivers license. Taking call may be required |
