1. Provide direct primary and acute care services to adult and/or pediatric patients as part of the primary care interdisciplinary team in the ambulatory department
2. Perform initial and on-going assessment of patients medical, physical and psychosocial status;
3. Obtain and evaluate health and medical history; perform physical exam, conduct preventive screening procedures based on age and history; Identify medical and health risks
4. Complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-up
5. Formulate appropriate differential diagnosis based on history, physical examination and diagnostic testing
6. Identify needs of the individual, family or community and develop a plan of care and/or education plan
7. Order, conduct, and interpret diagnostic tests, therapeutic treatments; may prescribe, administer and dispense therapeutic devices and legend drugs
8. Make appropriate referrals to other health professionals and community agencies
9. Reassess and modify the plan as necessary to achieve medical and health goals
10. Contribute to on-going medical and nursing staff teaching programs
11. Develop a patient education plan relevant to the condition and patient/family needs
12. Participate in performance improvement activities as assigned
13. Adhere to productivity standards established by the department
14. Assess need for personal growth and development and take the initiative in seeking opportunities to meet this need
15. Foster a service that is consistent with the family-centered care and shared management philosophy of the department
16. Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
17. Promote effective working relations and work effectively as part of a department/unit team interdepartmentally to facilitate the departments/units ability to meet its goals and objectives.
18. Perform other related duties but only after appropriate training
Physician Assistant Applicants:
? Graduation from an accredited physician assistant (PA) program with experience in providing care to adult and pediatric population
? AND at least 24 months of full time experience in orthopedics or a family medicine clinic
? Positions at the Hennepin County Medical Center are credentialed through the Office of the Medical Director in accordance with its policies.
Nurse Practitioner Applicants:
? Master s degree in nursing from an accredited program
? AND at least 24 months of full-time experience in the orthopedics or a family medicine clinic within the last 5 years as one of the following: Registered Nurse or Nurse Practitioner
? Possession of a valid license as a Registered Nurse issued by the state of Minnesota.
? Certification as an Adult NP or Family NP by an agency authorized to certify professional nurses in advanced practice
OR
? An approved equivalent as provided under M.S. 148/284.
? Positions at the Hennepin County Medical Center are credentialed through the Office of the Medical Director in accordance with its policies.
