A professional who:
Provides each patient with routine daily nursing care, including administering medications and performing treatments and procedures, and other desired services, in accordance with the patient's assessment, choices and care plan.
Is assigned to a primary household of 15 patients who are admitted for skilled rehabilitation or skilled nursing for complex medical conditions.
Is committed to patient centered care.
Must be an LPN or RN currently licensed to practice in the state of Minnesota. Must also possess excellent communication skills, the ability to build and maintain positive working relationships with patients and their families, physicians, peers, and other health care professionals, and be proficient in the use of electronic medical records systems.
Must be able to pass Background Study through the Minnesota Department of Human Services.
Work Schedule Information:
This posting is to fill a full time position, night shifts accruing 64 hours each bi-weekly pay period. This is a full time benefit eligible position.
