Job Description

The APP is a member of the CentraCare Complex Care Clinic multi-disciplinary team providing care for a diverse population of patients and families in multiple settings and will address all aspects of life that promote health and well- being. The providers main function is to assess, plan, evaluate, implement, and coordinate care of patients with complex and chronic medical issues in addition to preventative health visits under the direction of the Medical Director. Utilizes advanced assessment and physical examination skills to provide high quality patient care services requiring extensive skills and competency and serves as a resource to all patient care providers. Functions within scope of practice as defined by certification/licensure standards and state and federal regulations. Supports and implements patient safety and other safety practices as appropriate. Maintains professional competencies by attending educational programs, participates in professional organizations, subscribing to and reading professional journals or other appropriate activities. Supports and demonstrates Family Centered Care principles when interacting with patients and their families and with co-workers.

This new model for CentraCare coordinates care across the health care spectrum and is available to patients by professional referral. Clinic services include close medical case management with intensive attention to behavioral and social determinants of health. Multi-disciplinary team will organize and facilitate care within the health system as well as the community including the local jail system. CentraCare's goal is to improve the health of individuals who have not been successful in the traditional medical model.

Nurse Practitioner-Provider/Clinical Nurse Specialist

EDUCATION
Graduate from a school of nursing required. Masters Degree in Family Medicine Nursing with formal training as a nurse practitioner in the same specialty area of practice in which they are certified.

EXPERIENCE
Previous experience as a Nurse Practitioner preferred. Knowledge of nursing principles, policies, and procedures. Experience in an urgent care setting or emergency room desired.

SPECIAL REQUIREMENTS
Current licensure as a registered nurse in the State of MN required.
Current licensure as an APRN (Advanced Practice Registered Nurse) in the State of Minnesota required.
Certification as a Family Nurse Practitioner required.
Current American Heart Association (AHA) BLS Healthcare Provider card required per established policy.
Written agreement (prescribing agreement) with a physician based on standards established by the Minnesota Nurses Association and the Minnesota Medical Association in a Memorandum of Understanding that defines the delegated responsibilities related to the prescription of drugs and therapeutic devices.
Successful compliance with Minnesota Law, Chapter 372 relating to sexual exploitation of patients by staff providing psychotherapy services.
Successful completion of medical staff office credentialing must be completed prior to working without a preceptor.
Successful completion of third party payor credentialing must be completed prior to billing for services.

Physician Assistant-Provider

EDUCATION
Graduate from a physician assistant program required.

EXPERIENCE
Previous Physician Assistant experience preferred. Demonstrated skills in organizational management, leadership, communication, and clinical expertise. Experience in an urgent care or emergency room setting desired.

SPECIAL REQUIREMENTS
Current Physician Assistant licensure in the State of MN required.
National Commission on Certification of Physician Assistant (NCCPA) Certification
Current American Heart Association (AHA) BLS Healthcare Provider card required per established policy.
Successful completion of third party payor credentialing must be completed prior to billing for services.
Successful completion of medical staff office credentialing must be completed prior to working without a preceptor.
