Job Description

The Nephrology Advanced Practice Provider will provide comprehensive health care, under limited supervision of the collaborating physicians. The Nephrology provider practice includes, but is not limited to, comprehensive health care assessment, diagnosis, initiation of appropriate care and treatment of a variety of responses throughout the adult care continuum, and continuous evaluation of outcomes. The position includes medical domain of care. Will manage acute and chronic illnesses and promote wellness. Role models and supports a participative environment. The Nephrology provider practices within the scope of practice as defined by certification/licensure standards and state and federal regulations.

The Nephrology Advanced Practice Provider assists nephrology physicians and is responsible for providing care to patients at our outreach/dialysis sites, patient rounding at the St. Cloud Hospital, and clinical practice duties. The Nephrology Advanced Practice Provider also participates in anemia management, hypertension management, CKD education classes (as needed) and will facilitate the care of patients with End Stage Renal Disease/dialysis, CKD (chronic kidney disease), renal transplants and other nephrology related diagnoses. Maintains professional competencies by attending educational programs, participates in professional organizations, subscribing to the reading professional journals or other appropriate activities.

Nurse Practitioner Qualifications

EDUCATION
Graduate from a formal program approved by a national accrediting agency and be certified by a national certification organization recognized by the Minnesota State.

EXPERIENCE
Demonstrated skills in organizational management, leadership, communication and clinical expertise required.
Previous experience as a nurse practitioner is preferred.
Two year clinical (adult medicine) experience is preferred.

SPECIAL REQUIREMENTS
A valid Minnesota driver's license is required.
Must have advanced practice prescriptive privileges.
Current licensure as a registered nurse in the State of Minnesota required.
Current certification by a national certification organization recognized by the Minnesota State Board of Nursing.
Current licensure as an APRN (Advanced Practice Registered Nurse) in the State of Minnesota required.
Successful completion of medical staff office credentialing must be completed prior to working without a preceptor.
Successful completion of third party payer credentialing must be completed prior to billing for services.
Current American Heart Association (AHA) BLS Healthcare Provider card required per established policy.

Physician Assistant Qualifications

EDUCATION

Completion of a formal physician's assistant program approved by a national accrediting agency and be certified by a national certification organization recognized by the Minnesota Board of Medical Practice.

EXPERIENCE

Previous experience as a Physician Assistant is preferred.

Demonstrated skills in organizational management, leadership, communication and clinical expertise required.

Two year clinical (adult medicine) experience is preferred.

Must have a current Physician Assistant (PA) license in Minnesota.
