The Advanced Practice Provider is a member of the primary health care team who works in collaboration with Heartland physicians. The Provider follows established protocols and collaborates with physicians, however, does have day-to-day independence in health care decisions. This position will be located at our CentraCare Health Plaza.
The Provider has the option to work a schedule ranging from 64 - 80 hours every two weeks between the hours of 8:00am-5:00pm, Monday-Friday.
Provide health maintenance activities for new and established patients, to include performing history and physical exams while ordering appropriate lab and other diagnostic tests
Provides health care for a variety of injuries and illnesses, including stable chronic illnesses. Evaluates implements and manages patient treatment based on assessment of needs. Initiates treatment via prescriptions, diagnostic tests, exams and procedures.
Reviews lab results and plans actions according to results
Education for patients
Masters degree in nursing with formal training as a nurse practitioner in the same specialty area of practice in which they are certified OR graduate from accredited physician assistant program required.
Previous experience as a Advanced Practice Provider preferred
Current licensure as a Registered Nurse in the State of Minnesota required.
Certified as a Family Nurse Practitioner or Clinical Nurse Specialist
Minnesota Physician Assistant license required.
National Commission on Certification of Physician Assistants (NCCPA) Certification required.
