Ridgeview Medical Center
This position is responsible for providing nursing care in accordance with the Standards of Nursing Care and Practice as defined by Ridgeview Le Sueur Medical Center and the Minnesota Nurse Practice Act.
Collects and monitors patient health data and provides patient care.
Advises and educates patients, family members, and significant others on health maintenance and disease prevention for the patient.
Documents and maintains accurate medical records and reports.
Serves as a patient advocate in seeking appropriate community resources to meet patient needs.
Demonstrates competencies in the specialty area of Long Term Care.
Performs other duties as assigned.
When functioning as a Charge Nurse:
Manages flow of patients through inpatient and/or outpatient departments.
Assigns resources to care for patients and arranges staffing to ensure safe patient care.
Provides work direction and addresses issues during the shift.
Acts as a liaison between staff, patients, and physicians.
Demonstrates unit specific Charge Nurse competencies.
Reviews physician orders.
When functioning as a Primary Nurse:
Completes weekly charting.
Participates in care conferences in absence or as directed by Manager.
Completes the following monthly reviews: Care Plan, Restraint, Medication Self Administration.
Completes TD screen.
Complies Survey and Cart audits.
Updates master assignment sheet/walk list.
Participates in MD rounds in the absence or as directed by Manager.
Minimum Education/Work Experience/License/Certifications
Graduate of an accreditation school of practical nursing
Current Practical Nurse licensure in Minnesota or if licensed in another state, or a permit from the Minnesota Board of Nursing while the license application is in process
Healthcare CPR Certification
Demonstrate critical thinking and decision making
Ability to work autonomously and as a team
Capable of making independent decisions with the use of good critical thinking and problem solving skills within the scope of practice
Ability to work and multi-task in a high stress and fast paced environment
Ability to function both independently and as part of a team
Ability to respond appropriately to unpredictable situations and diverse patient populations
Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above
Previous nursing experience in a geriatric or nursing home setting
