Job Description

A bachelor's degree in social work, human services, or related field is required, and two years of professional experience in direct client assistance is preferred. However, an equivalent combination of applicable education and experience will also be considered. Must also maintain a valid driver's license and be able to pass a background check through the MN Department of Human Services.

Essential Duties and Responsibilities:
Assist aging adults and their caregivers with health-related, social needs to leverage and coordinate community-based services to promote, improve and maintain health and to provide support for great self-sufficiency
Conduct a comprehensive analysis of needs and provide coordination of services
Develop person-centered, individualized referrals, customized action plans, and client advocacy
Conduct face-to-face visits with community resource organizations
Collaborate with clients to ensure that they are meeting their goals and to offer support
Develop and maintain relationships with area service providers
Continuously monitor and assess the adequacy of referral agencies and delivery of services
Maintain and update resources in the community resource inventory
Organize community education events
Coordinate delivery of The Connection's marketing materials to key locations and partners, and represent The Connection at community outreach events as needed

Work Schedule & Benefits:

This is a full time (64 hour per pay period) position. The typical schedule will include daytime hours with occasional evening and weekend hours to attend and organize events or to conduct meetings.

Full time positions include: health, dental, life, long-term disability and retirement plan options.
