Ridgeview Medical Center
This position is responsible for providing nursing care under the supervision of the Registered Nurse in accordance with the Standards of Nursing Care and Practice as defined by Ridgeview Medical Center and the Minnesota Nurse Practice Act.
Collects and monitors patient health data and provides patient care under the direction of the Registered Nurse.
Conducts a focused assessment of the health status of an individual patient through the collection and comparison of data to normal findings and the individual patient's current health status, and reports changes and responses to interventions in an ongoing manner to a registered nurse or the appropriate licensed health care provider for delegated or assigned tasks or activities.
Reinforces previous education provided to patients, family members and significant others on health maintenance and disease prevention for the patient under the direction of the Registered Nurse.
Documents and maintains accurate medical records and reports.
Serves as a patient advocate in seeking appropriate community resources to meet patient needs.
Performs Intake duties as needed.
Demonstrates competencies in the specialty area of Home Health Home Care.
Performs other duties as assigned.
Minimum Education/Work Experience/License/Certifications
Graduate of an accredited school of practical nursing
Current Practical Nurse licensure in Minnesota or if licensed in another state, or a permit from the Minnesota Board of Nursing while the license application is in process
Current Healthcare Provider CPR certification
Must be at least 21-years old in order to meet Ridgeview's insurability requirements
Valid MN driver's license and meets Ridgeview's insurability requirements

Previous nursing experience in a hospital or similar healthcare setting
