Job Description

The MDS Coordinator is accountable for coordinating and overseeing the full collaborative, interdisciplinary assessment and care planning process in the Long Term Care Center. This process includes comprehensive resident assessments, care coordination and planning. Adherence to the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) requirements and collection and transmission of data for the purposes of quality improvement.

Responsibilities:
Provides and documents patient / family education based on diagnosis, treatment plan or prescribed medical regimen
Start Medicare coverage for newly qualified residents and assist with determining Medicare criteria for continued care through CMS guidelines
Completes accurate assessments within the RAI and MDS requirements and with proper coding
Completes Nursing sections of MDS and ensures completion of assigned sections by other disciplines in a timely manner to ensure compliance
Uses the MDS to create a personalized resident centered care plan
Completes comprehensive assessments per state and federal guidelines
Assesses patients condition based upon observation, examination, and/or interview, laboratory findings, and other pertinent information
Uses professional judgment and critical thinking to prepare, prioritize and execute the patients plan of care consistent with patient assessment data and physicians treatment plans and professional standards of evidence-based practice
Implements nursing interventions to achieve progress toward expected patient outcomes or to stabilize a patients condition and/or prevent complications
Evaluates and documents patients response to nursing care and modifies or revises treatment plans, and / or contacts physicians as circumstances require

Qualifications:
Current RN license with Minnesota Board of Nursing, in good standing
Current BLS Certification(or willingness to obtain within 6 months of hire, company paid)
Preferred experience in Geriatrics
Preferred experience with RAI process, Medicare Part A, Case Mix, Federal and State regulations

Schedule:
80hrs/2wks
